Logging in to Bridge
Who this is for
- Pathfinder Bridge Users. (Staff members and care teams of an organization)
Overview
The Bridge log in supports two different methods for staff to choose from:
- Logging in with a code
- Logging in with a password
Common Issues include:
- Not receiving a code, Forgot Password, not a user within Pathfinder.
- If an admin or super admin has not added you to the Pathfinder system, you will not be able to receive a code or reset your password until you have an account.
- If you are not receiving a code, oftentimes it is located in your spam or junk folder in your email.
- If you have forgotten your password, click on the forgot password link and you will receive an email with instructions on how to set up a new password.
Log in with a code
- Go to https://bridge.pathfinder.wellstoneapps.com/auth/login
- Select the “Login with code” button
- Enter your email address that is in the Pathfinder system. This is often your work email address.
- Select the “Get Code” button.
- Check your email for a message from Pathfinder containing a code. Sometimes this email can take up to a minute to receive. If you are not seeing your email be sure to check your spam/junk folders.
- Enter the code that was provided and select the “Submit code” button.
Log in with a password
- Go to https://bridge.pathfinder.wellstoneapps.com/auth/login
- If you have not set up a password or forgotten your password select the “Forgot password?” link.
- Enter your email address that is in the Pathfinder system. This is often your work email address and then select the “Continue” button.
- Check your email for instructions on how to set a new password. If you do not see this email, be sure to check your spam/junk folders
- Enter your email address that is in the Pathfinder system. This is often your work email address. Then enter your password and select the “Submit” button.
Troubleshooting
- If an admin or super admin has not added you to the Pathfinder system, you will not be able to receive a code or reset your password until you have an account.

- Your email address originally entered in the system is incorrect.
